Summary
This position provides medical case management to persons infected and affected by HIV/AIDS within a four-county area in Florida (Indian River, Martin, Okeechobee, and St. Lucie Counties). The Human Services Analyst completes comprehensive needs assessments, develops individualized care plans, monitors client progress, and coordinates linkages to medical and social services. The role reports to the Operations Management Consultant II within the Florida Department of Health's HIV/AIDS Program.
Key qualifications
- Ability to complete comprehensive needs assessments and develop individualized care plans for HIV/AIDS clients
- Knowledge of Medicaid, Medicare, VA benefits, and Ryan White services eligibility and enrollment processes
- Experience with case management documentation systems such as CAREWare and HMS databases
- Ability to provide treatment adherence support and coordinate linkages to medical, housing, substance abuse, and mental health services
- Strong client advocacy and communication skills for face-to-face assessment and ongoing monitoring
